05' CR run rough at slow speeds

we got a 05' common rail and stick and taking off in 1st and 2nd gear it falls on its face and tries to die , but once we get goin say upwards of 40 mph it runs fine, we replaced 2 injectors that went bad in it and it ran fine for about 30k miles afterwards and now this, could it be my cp3 going bad or has anyone else had this problem, i was even thinking a possible hole in the fuel line but its not dripping or leaking anywhere and when it stumbles and tries to die there is no smoke..

sounds like a fueling issue......whats yer fuel pressure during all this?

L/P are know to be troublsome and this is classic symptoms
