Shift it and turn the input, count the turns on the output yoke. You can also pull the top cover and take a look, see if there is still oil in it then pull the plug on the bottom and get some oil out see if it smells good and burned. That is a about as much checking as You can do when buying one, outside of taken it apart.

Well when You're in 4th it should turn the same speed as the input, all other gears will be slower.

T19's are pretty sturdy transmissions, but anything can get worn out. Look for the things EZ posted above, and have a good look at the trans to t-case adaptor. It's aluminum and in abused rigs, it can be cracked. The T-case is likely a BW 1345 rather than a 1356. Either will bolt to the rear of the T19 though.

If the tag is gone, look at rear of the T-case. If it has a bolt on rear cone and a slip yolk or if it's got a PTO port, its a 1356. If the rear is cast as part of the T-case and there is a u-joint yoke held on with a large nut, it's a 1345.

f250 1356's don't have the PTO port.
f350 1356's have the PTO port.
BOTH HAVE removable tailcones. 4 bolts and the tailcone comes off.
1345's have an intergrated rear half of the case and tailcone as one unit.
