Idler Pulley/ Serpentine Belt

All,
I am looking for a solution to the serpentine belt moving back and forth on the Idler Pulley. I have already had the belt slip off twice. This is a new pulley/bearing. Any suggestions? It is moving back and forth and towards slipping more than I think it should.

Something upstream may have a bad pulley. How are the rest of them? Any wobbling pulleys?
