On the torque convertor lockup...the first thing to do is to see if the computer is commanding lockup...yes...or no....

If yes...why..? is the TCM seeing some parameter that encourages it to command lockup..?

If no...then there is something stuck in the pump or valvebody that is forcing the convertor clutch to apply...or...there is an internal leak in the pump or convertor that will not supply release oil to force the convertor clutch off...

You see...the clutch applies going forward...towards the front of the convertor...and as soon as you start it up...the engine...the pump supplies oil to the front of the convertor to release it...and when the clutch apply system is enabled...the release oil is drained and apply oil ramps up...

So...unless the clutch is applied when you bump it in gear...thereby stalling the engine...my bet at this juncture with the limited info I have to work with...is that the TCM is commanding lockup...

Of course I have been wrong before...just ask Jesus Freak...

On the no 2-3 upshift...here again we have the question...

Is the TCM commanding an upshift...or not....

If yes...there is a 2-3 upshift command...oops...no Direct Clutch left to shift into...or...stuck valve in the VB or bad shift solenoid or connection to the solenoid...

Does it have Reverse..? you need Direct Clutch to apply reverse...

If yes...then breathe a little easier...the problem might be simpler than you are thinking...

If no...then the TCM is holding back the upshift command...because it sees some parameter that makes it think that you are not ready for a 2-3 upshift...

What is needed here is a rudimentary 'signal monitor'...an electrical device with LED bulbs tapped into the solenoid wires that would light up when the solenoids are commanded...one LED for each solenoid...

ATRA mapped out a device for that purpose for its members a number of years ago and it was simple to build...you just needed a Radio Shack that was helpful in gathering the components...

We used a radar detector mount with suction cups to mount it to the windshield..and ran the wires near the main harness connector at the unit...

Just a thought...you replaced the road speed sensor...right..?

Did you check and see if the tone ring on the rear carrier is still there to excite the sensor..?

And...sometimes the sensor connector is not making good electrical connection...there was/is a pigtail harness to replace the bad connector...

If the TCM is not seeing any road speed...it doesn't see any reason to command any shift...

Just my .02...

Pull the connector off of the solenoid pack. Check to see if there is fluid in the connector.
A malfunction in S1 and S3 will cause a no 2-3 shift and converter lock up issues.

89 can be a real pain to get codes from. Sometimes there are stored codes. Most of the time they show up after the unit fried.
