I have a 1992 F-350 with a 7.3 IDI. The injector caps and o-rings need to be replaced, and the driver side went fine; however, I am having trouble with the passenger side. On cylinder #1 there is something screwed into the top of the injector that has the cap captured. I can't seem to get it to unbolt. I can not find a fuel diagram that has this part on it. Since I am new to diesel engines, I thought I would post a picture and see if anyone can help with what this is. Thank you in advance for any help with this.

It looks like a hack someone did because the steel line got bent or damaged and maybe they cut off a bad spot on the steel line, flared the end and put that extender thing to make it reach. In theory, it should just come off. Try putting a wrench on it and hitting the wrench with a hammer to break it loose.

That is the pickup for the Rotunda style (I think) timing meter. You need to remove the injector line first, then that adapter. After that, you'll be able to remove the cap. You may have to hold a wrench on the line nut and on the adapter if they both want to turn. They are both 5/8".

Yep. Mine has one too. Came with the truck. When I went to replace the return lines, it confused me too.

Pull the steel line with a 5/8 wrench as usual.

Then there is a plastic sleeve that you can pull off.

Then what is left is also taken off, just like the steel line was, using a 5/8 wrench.

Put it back, when finished. I believe the line has been sized, to have that inline. (I believe the lines are all pretty precise in length. If you change that you might alter the performance.)
