Rough idle, runs great warm

I have a SD 2000 .(115 k) It has been starting great fires quickly. If I drive off before it is warm it will die and start real hard. It also runs really rough until it is completely warm. It sometimes has no throttle when cold.Used to kick into high speed idle, doesn't seem to anymore. I replaced the Camshaft sensor , new fuel filter when this started a week and half ago.
