I had that issue on my 6.9. Checked everything else then put a rebuilt pump and new injectors. Problem solved! I think sometimes as the pump and injectors wear it is so gradual we don’t notice. There was a huge increase in all around performance after the pump and injector rebuild.
I have on on an 86 6.9 2wd with 3.55’s I have the OD switch on the shifter. Did away with the auto shift since it always “thought” it should be in OD at a dumb time. Used a control box from Maximum Overdrive. Still has the minimum mph cut out feature to protect the clutch.

No pics but last Sunday I hauled 2 loads of big preggo cows. About 2,000 lbs a cow so it put me over 20k per load I imagine. 1.5 mile 7% grade finds the weak spots. Usually pulls it in 3rd but pulled down to second while holding good fuel pressure and very little smoke so some checking to do!
On the CA smog. All of the F-Superduty trucks in CA were killed off by CARB. So if it came out of a FSD the story could true. No other IDI trucks were taken out of service in CA except the rare truck that smoked really bad and wasn’t fixed.
One really quick and nearly foolproof way to tell a turbo block is the port in the top rear that on a NA engine has the oil pressure sender is 1/8 npt and on a turbo engine is the turbo oil supply and 1/4 npt.

There is a Snap On MT-257 timing tool with a Snap On dual back timing light on eBay right now. $260 shipped is a little rough but it is everything together. FYI....I already have one.
That’s a MT257. Much simpler than the MT480. The 480 uses a luminousity probe in the glow plug hole to “see” the combustion. And a magnetic pickup for the timing reference.
