I concur with Scott - dose the heck out of her. I have had good luck with Stanadyne branded products in the past. Only problem is they are only sold typically through injection pump shops. I know others have had good luck with Diesel Clean as Scott mentioned.
I noticed from your screen name that you are running waste motor oil - has it been fully filtered before you dumped it in you tank. The reason I say this is that I used to run alt fuels and if I didn't get them fully filtered it would plug a filter pretty quick.

what transmission does it have? If electronically controlled (E4OD) it maybe the relay that powers the PCM. I have had this once. How I found it was tracing the wires that had current flow to them to a relay on the drivers side fender that was welded shut (contacts had burnt spots on them).
You have air getting into you fuel system. Typically this is caused by an o-ring failure on the return line cap system on the injectors. Which means it's time to replace. You can approach this at a couple of different ways. You can replace only the o-rings and caps that are giving you problems...
