What is the difference between the T3 or a T4 flange on a replacement turbo?

The size of the exhaust housing mounting. T4's are bigger.

Ok, Do I need to change the exhaust manifold on my 5.9 12 v for a T4 or get an adapter?

You will need to get an adapter. T3 - T4 adapters are pretty common.
Although I'm sure you could probably find an exhaust manifold with a T4 mount on it somewhere online.
