Need advice

I have a couple of issues that are bugging me a bit.

1. When I start my truck cold it seems as if I have air intrusion and have to crank for about 5-7 seconds get the air out. BUT, when the truck starts it puffs out a HUGE fairly continuous cloud of blue smoke and idles rough for a while (10 seconds). An increase in RPMs clears this idle issue up faster. I would think because of the smoke it is getting fuel but just not firing off right away. I have replaced the glow plugs and return caps and both seem to work as intended. Injectors causing this? Truck has 142,000 on the clock.

2. I have a Holley Red installed and at idle there is a HUGE amount of fuel coming out of the return port on the IP with the hose off. I assume this is because I have the Holley and not the factory lift pump that is regulated by RPM. Correct? I am taking Mel's advice and running a dedicated return for the IP and filter housing.
