Use any pre 95 solenoid pack. New, rebuilt, what ever. It has to be pre 95. Otherwise it will not work.

Yes check the accumulator body for a stuck valve. Sometimes over tightening will cause issues like stuck valves.
There could be an internal issue not allowing the tranny to shift to forth gear.
