Triton 5.4 blown plug 2 times

Ok so my neighbor has a E250 with like 90k miles and last year he blew a plug on #3 so he took it into a local shop and had it replace well Sunday he called me asking for help he br Ok down like 70 miles away he blew a plug again so I tow him home and today he tore onto the van and found that the helicoil/ insert installed blew out has anyone had this happen and how did they fix it

A company called time sert makes bigger inserts. The helicoil kits never work on these. Had one the other day that that particular hole had been messed up by a couple different repairs, ended up taping the plug hole for 1/2 npt and building an insert to fit that......
