They are never meant to be used on a manual trans. The ONE time you forget to leave it in neutral....well, welcome to prison and bankruptcy.
I stand corrected, but how does it set the fast idle and cold start advance?
Cold start advance turns itself on.
High idle isn't a necessity, but it does help.

I took the high idle off my 6.2 when I swapped it in.
Started and warmed up just fine. That engine had a flexfan.

In all seriousness, get one of the "throttle control cables."
Can't remember the exact name. They're used a lot on older big trucks.
You twist it out to bring rpm up, then push in the center to let it down.
