I have all this information in my International Engine Service Manual exactly as you have posted here. My manual is listed as a CGES 435-1 dated July 1991. They are still available from International for around $60.00... Well worth the cost. Just read up on how to install the rear main seal.

Why do cylinders 7 & 8 have a different cylinder bore diameter?

The reason for that is the cdr attached to the intake manifold at the back and the angle the engine sits at. All the oil thats sucked into the intake manifold runs down to the rear most cylinders 7 and 8. So those cylinders get more heat from burning more fuel. Oil from the engine is fuel to these diesels. Now nobody will bore these engines like that either. Its only around 1/2 a thousands larger anyway.
