If you read your battery voltage correctly you need to charge your batterys as they are sitting at about 50% charged. Not saying you may have other issues but that would be a good place to start. Fully charged you should see 12.6-12.7 range.

Heat the plug until it is red with a torch then take a blow gun and blow in the center of the plug for 30 seconds. The plug will cool faster the the water pump housing and usually the plug will screw right out without any issues.
Well there goes all those ideas! If the flywheel is truely flat and not warped and you have free play I would suspect you have some weak springs in the pressure plate. What else could it be?
