I have a 2wd 4 popper 5 speed ranger too, 25mpg daily driver, and I hate it too LOL Picked it up for almost nothing and have put about 40k on it with barely any issues though. Its always nice to jump in the diesel or the mud truck and actually have some room.
If equal pressure were going to both ports it would still extend. The area of the piston on the extend side is larger than the retract side due the the rod taking up the extra space on the piston.

I would have to guess what caused the failure was piston/valve contact due to the floating valves. The bottom picture is the pushrod showing the curve in it. That said I put a new bolt and 2 pushrods in and drove it like that for a long time. Never tore it down to see how extensive the damage...
