I can't find the next 5 pics.Based on the first one,my guess is,the lug nuts loosened,once the outside tire tipped inward,it rubbed on the inside one,which heated it up and caused it to blow.Then the outside tire went wee,wee,wee,all the way home.:D

Did you find it? Everyone ok?

Wow! The studs all snapped off.So the nuts were loose for a long time? Resulting in the rims lug holes getting reamed out,then the inside tire goes flat,putting all the weight on the outside tire,the studs and say uncle and then the tire takes a vacation from the short bus.:D

Oh no wait.I see what the problem is.Looks like a wheel adapter back there.Notice it's not hub-centric.This is the reason why over time those holes in the rim got reamed out.It also explains the hub ware groove there.See it? Now what happens is the wheels hop and the rims holes ream out even more,now all this back and forth pressure on take off and stop is putting stress on the studs and eventually......That's all she wrote.These studs snapping and the outer wheel leaving the bus at the same time the inner tire blows is a strange and unfortunate occurrence.

No adapter, it is hub centric.

My guess is that the inside tire that picked up the screw went flat first, and the vibration loosened seven of the nuts, allowing the outside wheel to lever off the remaining tight nut, hence the one round hole.

That thing weighs 11.4K#. Getting it on a flat bed was a lot of fun, in light snow fall.

Right after we got it fixed I jam nutted four bolts on each wheel.

It looks like a wheel spacer rather than a wide enough rear axle,however I guess it doesn't matter so much,so long as both rims can fit inside on the hub center,before it tappers.Zooming in,it does look like they do.

I've driven with flat tires on the inside to drive to a tire shop without problems a few times.No vibrations but I know to drive her easy until I get it fixed.The whole sidewall blowing out like that,that has to be caused by heat/tires rubbing no? I suspect you had loose lug nuts for whatever reason,long before the flat.

99+ Duallies have a rear hub that looks like the fronts on a 97 down trucks. I was helping a buddy with an 01 dually and was surprised also...... No spacers, just a brake rotor that sits back away from the lugs....

That happened at 70 MPH on a freeway, in the Tehathapi Mt curves, with no shoulder. I have had a few flats at speed and they all looked like that by the time I got them stopped.

Had a similar experience driving my shuttle bus home to Denver from North Dakota... almost home and inside tire went flat and started gnawing away at the studs. About 45 miles from home major vibration and when I got it pulled over 7 of 8 studs gone. Good news was daughter was behind me so I had a ride...bad news was my daughter was behind me and I thanked the good Lord to this day that it didn't end worse.
