It’s definitely making me wonder if I’m getting bad data like you guys mentioned.
Alright, so you've managed to make a modern vehicle out of your ancient vehicle by putting so many sensors on it that you have a "bad sensor" thus your "check engine" light is on and your concerned about a problem that might not exist. I'm being silly, but you know that's funny right there!

Hey I don't want to hear it. Redundant sensors for your redundant sensors are key in diagnosing things

As sad as that sounds.... It's true lol
But it's redundant. On our ancient trucks you want to keep the temperature gauge around "R", "M" is okay, but if it gets too far into "A" you get concerned, and if the gauge makes it to "L" you need to pull over and look at something. NOW THAT'S! redundant diagnostics!

Just throwing this idea out here because I have had it happen on a 302 before. Does the rotation of your water pump match your drive belt setup? On the 302 the parts store gave me a v belt pump for a serpentine setup.

I think I found it…
Went to vacuum fill today. Thought to myself, why don’t I check for the spring in the lower rad hose. Check this out.

You know when I said earlier, “it almost seems like the coolant sensor isn’t measuring coolant and is just measuring the temp of the heads. That’s correct. It was just measuring the temp of the heads.

Also, confirmed, you can drive an IDI for miles and idle it for 4 hours and it won’t overheat with no coolant flow.

That's awesome! Praise the Lord you checked, that could have taken a while to find. If you mess with old tractors like I do from time to time, and the tractor you score has been sitting for a while, and if the cooling system is open ANYWHERE try to check for mouse nests. Mise will nest inT the block and radiator. Their nest will get soft travel to the thermostat and clog it up good. Lol way to go, Hacked!

Dang you're lucky! I'm currently doing a head gasket on a 6.7 Cummins that had something similar done to it.. Rag shoved in the water suction line (lower rad hose) and it got up to 260° for about 4 minutes, popped the head gasket. Looks like you dodged a bullet.

Sabotage I tell ya! What could have that been from?
Ironically I said the same thing. But the more realistic answer is someone probably capped it to keep debris from getting in there for whatever reason, and forget about it.

Looks like a cap from a Grease Cartridge.
Them dang Grease Monkey`s will do that every time.

I woda never thunk of checking the lower Hose.
Put this in my bank of info.
